Unit Mission
The Health & Medical Support Community Unit provides community health and medical support — developing the health outreach programs, medical assistance coordination, and community health worker systems needed to improve access to care for underserved populations. The unit applies community health, health equity, and primary care to develop the grassroots approaches that bring health services to communities where access is limited, addressing the social determinants of health and building community health capacity.
Key Functions
Health Outreach
Conducting community health outreach — deploying community health workers and mobile health services to bring health education, screening, and care coordination to underserved communities.
Medical Assistance
Coordinating medical assistance — connecting community members with free or low-cost medical care, prescription assistance, and specialist referrals, removing financial barriers to health access.
Health Navigation
Providing health navigation services — helping community members navigate complex health systems, understand their health conditions, and access the services they need for effective health management.
